CLINICAL NEUROSCIENCE ASSOCIATES, INC
CNSA Neurology (opening Spring 2018)
Jon M Gustafson MD MBS
We are logging messages coming in by phone, fax and email. The schedule database is being created and when able we will begin to contact patients to schedule appointments. The demand for appointments is very high. We are expecting significant delays for many patients even when appointments become available. Priority is being given to previously established patients (previously seen in a clinic by Dr Gustafson).
Patients and referring physicians are reminded this is an entirely new practice and while Dr Gustafson has practiced in Fort Smith for many years, federal regulations require creation of an entirely new chart. Any transmission of records from prior clinics will need to comply with HIPAA regulations including documentation of patient permission. We know this creates significant difficulties for everyone, but this result is due to federal health regulations and actions taken completely outside our control.
Physicians and clinics - communication by fax is best HIPAA compliant technique at this time. Please use 844-454-8351 for now. We expect an update of this information in the next few weeks.
Refills and pharmacies - please send requests for refills via fax to 844-454-8351. We will process these as able and may be limited by availability of records.
Health Plans - the process for "accepting" a health plan follows these steps - under consideration, credentialing, enrolling, verification, "accepting". Various plans and their statuses are listed below:
Blue Cross - under consideration
Cigna - under consideration
Aetna - under consideration
Humana - under consideration
United Health - under consideration
Medicare - enrolling
Medicaid Arkansas - under consideration
Medicaid Oklahoma - under consideration
Tricare - under consideration
If you wish to have our practice consider your health plan, the best method is to contact your health plan and ask them to contact us and make a proposal for our consideration.
Payment Methods - for patient responsible amounts we accept cash and checks. We expect to be able to accept major credit cards and will be considering the various e-payment methods now available. The patient portal, when activated, will be able to take payments.
Continue to follow this page as practice development moves towards our opening.
